About The Role

Chronius Health is seeking an empathetic and experienced Nurse Practitioner to join our first-of-its-kind virtual-first primary care team, focused solely on patients living with or suspected to have invisible illnesses. As a Nurse Practitioner at Chronius, you will play a crucial role in delivering high-quality, personalized care to our patients, addressing both acute and traditional primary care issues. This role is part-time (~10-15 hours per week). There is potential for this position to transition into a full-time role.

Responsibilities:

  • Provide comprehensive primary care services to patients via our virtual-first care model, focusing on acute and traditional primary care issues such as minor injuries, infections, cold and flu symptoms, allergies, chronic disease management, and preventive care.

  • Develop personalized care plans for patients, considering their unique symptoms, medical history, and lifestyle factors.

  • Collaborate with our multidisciplinary team of healthcare professionals, including primary care physicians, mental health specialists, and nutritionists, to ensure a holistic and coordinated approach to patient care.

  • Actively participate in ongoing patient monitoring and adjustment of care plans as needed, in order to prevent complications and improve overall health outcomes.

  • Stay up-to-date with the latest evidence-based practices and guidelines relevant to primary care and invisible illnesses.

  • Educate and counsel patients on various aspects of their health, including lifestyle modifications, medication management, and self-care strategies.

  • Maintain accurate and timely documentation of patient encounters in our electronic health record system, adhering to all HIPAA regulations.

Requirements:

  • Ability to provide care to patients during East Coast hours.

  • Master's degree in Nursing from an accredited program.

  • Current active licensure as a Nurse Practitioner. Ideally, this includes licensure in the state of Florida.

  • Board certification as a Family Nurse Practitioner or Adult-Gerontology Primary Care Nurse Practitioner (NP, FNP, or A-GNP)

  • At least 3 years of clinical experience in primary care / family medicine / internal medicine, rheumatology, or a specialty that often sees highly complex, chronic patients. Ideal candidate experience will include a focus on managing acute and traditional primary care issues.

  • Strong communication, interpersonal, and organizational skills.

  • Proficiency in utilizing telehealth platforms for patient care.

  • Demonstrated commitment to providing compassionate, patient-centered care for individuals with invisible illnesses.
