Care Operations Coordinator (Contract)

Position: Care Operations Coordinator
We’re seeking a Care Operations Coordinator to support the day-to-day operations of care. Your role is to support frontline LCMs and enable them to provide the best care possible—whether that’s intaking new mothers, processing referrals, or helping us strengthen relationships with health plans.
Responsibilities
Outreach Operations Support – Follow outreach protocols to make sure each mother has been welcomed and enrolled
Referral Processing & Coordination – Use referral ticketing system to process referrals quickly and accurately, be persistent with consistent follow-ups to advocate for benefits the mothers need
Health Plan Relationship Support – Attend payer/health plan meetings, take notes, document process updates, and share clear summaries with the team
Operational Flexibility – Jump in wherever needed to support care operations; we’re a startup and we move fast!
Qualifications
1–3 years of experience in healthcare operations, patient coordination, or case management
Familiarity with referral workflows, payer processes, community resources
Bachelor’s degree in healthcare administration, public health, or related discipline.
Strong organizational and multitasking abilities
Comfortable working in fast-moving, tech-enabled environments (e.g., Notion, Slack, Google Workspace)
